How Do You Know It’s Sciatica?

Sciatica is pain that typically starts in the lower back or buttock and radiates down one leg, sometimes reaching the calf or foot. You might feel a sharp, burning sensation, numbness, tingling, or a deep ache that gets worse when you sit, stand for long periods, or bend forward.

Some patients describe it as a constant dull throb that flares with movement. Others feel sudden, shooting pain that makes it hard to walk or stand up from a chair. The pattern can vary, but the common thread is pain that follows a path from the lower spine down through the leg.

If any of this sounds familiar, you’re not alone. Sciatica is one of the most common reasons patients walk through our door.

What Causes Sciatica?

Sciatica occurs when the sciatic nerve, which runs from the lower spine through the hips and down each leg, becomes compressed or irritated. The most common causes include:

Herniated or bulging disc

This is among the most frequent reasons we see patients at Pulse Chiropractic. When a spinal disc shifts out of position, it can press directly on the nerve roots, producing radiating pain and sometimes weakness in the leg.

Degenerative disc disease

Age-related wear on the spinal discs narrows the space available for the nerve, gradually increasing pressure over time.

Spinal stenosis

A narrowing of the spinal canal that compresses the nerves passing through it; more common in patients over 50.

Piriformis syndrome

The piriformis muscle in the buttock tightens or spasms, putting pressure on the sciatic nerve that passes beneath (or sometimes through) it.

SI joint dysfunction

Inflammation or misalignment of the sacroiliac joint can irritate nearby nerve structures and mimic or contribute to sciatic symptoms.

Determining the origin of your sciatica matters because the treatment approach differs depending on the cause. That’s why every sciatica patient at Pulse starts with a thorough evaluation before any treatment begins.

How We Treat Sciatica at Pulse Chiropractic

During your first visit, Dr. Rill performs a thorough chiropractic and orthopedic evaluation. This includes postural evaluation, range of motion, orthopedic and neurological testing, and X-rays if needed. He will use this information to identify the origin of the nerve compression.

From there, your treatment plan is built around your specific diagnosis. Depending on the cause and severity, your plan may include:

Spinal adjustments: precise corrections to restore proper alignment in the lumbar spine, reducing pressure on the nerve root and improving joint mechanics.  Learn more about our Manual Therapies.

Spinal decompression: a gentle, non-surgical approach that creates space between compressed vertebrae, allowing bulging or herniated disc material to retract away from the nerve. This is especially effective for disc-related sciatica. Learn more about our Therapeutic Modalities.

Soft tissue therapy and cupping: targeted work on the muscles surrounding the spine, hips, and piriformis to release tension, improve circulation, and reduce nerve irritation caused by muscular compression.

Electrical stimulation (E-Stim): helps manage acute pain, reduce muscle spasm, and promote blood flow to the affected area during the early stages of treatment.

Intersegmental traction: gentle mobilization of the spinal segments to improve flexibility and reduce stiffness that contributes to nerve compression.

Rehabilitative and corrective exercise: once acute symptoms are managed, we build a movement program to strengthen the muscles that support your spine, improve hip and core stability, and reduce your risk of recurrence. Learn more about Sports Injury Care & Rehab.

When Should You See a Chiropractor for Sciatica?

Many patients wait weeks or months before seeking care, hoping sciatica will resolve on its own. Sometimes it does, but when it doesn’t, the longer it persists, the harder it can be to treat.

Consider scheduling an evaluation if:

  • Pain has lasted more than a week without improving
  • Radiating leg pain is interfering with sleep, work, or daily activities
  • You feel numbness, tingling, or weakness in your leg or foot
  • Pain worsens when sitting, bending, or standing for extended periods
  • Over-the-counter medications aren’t providing meaningful relief
  • You’ve been told you might need surgery and want to explore non-surgical options first

FAQ

Yes. Chiropractic treatment addresses the mechanical causes of sciatic nerve compression, misalignment, disc displacement, and muscular tension. Many patients experience meaningful improvement within the first few weeks of care.
It depends on the cause and severity. Some patients feel significant relief within 2 to 4 weeks. Disc-related sciatica or longer-standing cases may require a more extended treatment plan. Dr. Rill will outline your expected timeline.
No. Spinal decompression is a gentle, controlled technique. Most patients find it comfortable and often feel immediate relief in pressure and tension during the session.
It can if the underlying factors aren’t addressed. That’s why our treatment plans include corrective exercise and movement strategies designed to strengthen the supporting structures and reduce the chance of recurrence.
We accept most major health insurance plans and offer self-pay options. Our team provides a complimentary insurance verification before you start care so you know exactly what’s covered. Call us at 443-603-9252 or submit a request online.
Many patients come to us specifically to explore non-surgical options before committing to a procedure. A 2024 study published in the Journal of Contemporary Chiropractic found that patients treated with spinal decompression combined with chiropractic care achieved measurable reductions in herniated disc size and full recovery from pain, without surgery, confirmed by MRI follow-up (Dudum & Gatterman, 2024). That is the kind of outcome we work toward with every patient who walks in wanting to avoid the operating room.
